One thing for sure is migraine does not equal head One thing for sure is migraine does not equal headache! According to Harvard Health “It has been said that a migraine attack is like a walking on the moon- unless you have experienced it, there is no way to describe it.” 

We get so many symptoms that are difficult to explain to others. From phantom smells to Alice in Wonderland syndrome the neurological sensory symptoms we experience can feel very strange & sometimes even frightening.

An estimated 9% of men experience migraine attacks An estimated 9% of men experience migraine attacks. 

While physical symptoms of migraine in men and women are generally the same, top triggers can differ. Women are far more likely to experience a migraine attack resulting from hormone level changes. Men are more likely to trigger migraine attacks from physical exertion and lifestyle triggers. 

Men are far less likely to report their migraine condition and seek treatment, but they still experience debilitating symptoms. 

According to Harvard Health research has suggested that migraine is also associated with an increased heart attack risk and stroke risk.  One of the main Harvard studies that looked exclusively at the link between migraine in men and the risk of cardiovascular disease examined 20,084 men age 40 to 84 who participated in the Physicians' Health Study and completed detailed medical questionnaires each year to record their health status. It was reported that having migraine boosts the risk of heart attacks by 42%. A study in the journal Neurology found that migraine sufferers are twice as likely to have heart attacks as people without migraine. In that study, heart attack risk was higher among those whose migraine was accompanied by auras.

Other research from the Loyola University Health System suggests that people with migraine have about double the risk of suffering an ischemic stroke caused by a blood clot, the most common kind. It is not clear how migraine is associated with a higher stroke and heart attack risk, but one theory is that they may share common mechanisms.

🧠 According to Dr. Richard B. Lipton, MD “Though there have been many breakthroughs in migraine therapy, including drugs, devices and behavioral treatment, there is a wide gap between treatment available and treatment delivered.”

A few of the barriers in that treatment gap could be…

🧠 Restrictive insurance policies limit access to the very treatments that can prevent and treat these debilitating symptoms. 

🧠 Access to knowledgeable and “appropriate medical consultation” extends diagnosis wait times, prevents proper diagnosis and leaves no protection against medication overuse headache.

🧠 Doctors without proper knowledge of the debilitating aspects of migraine and migraine variants may treat migraine patients as drug seekers and refuse to write patients prescriptions for acute and abortive medical treatments that help. 

🧠 One example of this… I’ve heard my vestibular migraine specialist say he thinks it’s cruel to make vestibular patients go without vestibular suppressants like benzodiazepines. Yet, I talk to others diagnosed with vestibular migraine still suffering with daily dizziness who’s doctors refuse this effective rescue treatment.

🧠 Migraine is often misunderstood or dismissed 🧠 Migraine is often misunderstood or dismissed by the general public as “just a headache”. That stigma can impose adverse consequences on the every day lives of those with migraine. Bosses become irritated that we’re calling in sick with a headache and friends and family become frustrated that we’re canceling plans, yet again… over just a headache? We all know head pain is just one of the many debilitating neurological symptoms we experience with migraine disease.

🧠 Here’s the bottom line. We have a neurological condition called migraine every single day. Migraine comes with disabling symptoms, we must not allow others to trivialize that. No one knows better than those with chronic migraine that even if we are not experiencing an attack, we are fighting to suppress them daily. 

🧠 Hyper-sensitivities to light, sound, chemical/fragrance, hormones, weather, pain and more are always there waiting to trigger an attack. Because migraine is the name of our disease always… we don’t “get migraines”. 

🧠 Have you noticed that experts who speak at the @migraineworldsummit do not use the word “migraines” or “a migraine”? They used the phrase “migraine attack” or “migraine days”. Why? Because saying migraines is like saying fibromyalgias or epilepsies. 

🧠 When you stop to think about it isn’t it wrong that we allow the name of our disease to be used in the same context as a symptom of it? Saying you have “a migraine” sounds exactly the same as saying you have “a headache” and contributes to the stigma.  Are you still with me? Do you hear the difference?

🧠 The language we use around our disease is important. Stigma prevents proper diagnosis, research and funding! We deserve better! If we all make up our minds to adjust our language we can make a huge impact to change the stigma that migraine is just a headache and help others understand what migraine actually is… the name of our neurological disease. Effectively turning stimulus off and moving it into the background. But for those of us with migraine our brains don’t easily adapt.

🕶 The word to describe our light sensitivity, Photophobia literally means fear of light. I personally think this term misses the mark in describing what we experience with migraine. I believe just like hyperacusis our light sensitivity is a disorder in perception and processing of the light, and not a fear of it.

🕶 But, for now photophobia is the term we have to describe our hyper responsive migraine brain’s reaction to it. Please know when we use the term sensitive we’re not referring to a psychological or personality trait, we are talking about our brain’s neurological reaction to stimulus.

🕶 A range of light from bright indoor lights to dappled sunlight or strobing lights can feel unbearable and even painful for those with a migraine.

🕶 Not only are we sensitive to it during an attack, but just like with noise, movement, smells and physiological stimulus like hormones, hunger, pain etc, light can be a trigger resulting in a migraine attack as well.

🧘🏽 Exercise gets a bad wrap  because of conflicting opinions about whether exercise will make your symptoms worse, it’s often thought of as a trigger. But, that reputation is largely undeserved. 

🧘🏻‍♀️ Movement really is a crucial part of migraine prevention. Research shows that regular exercise can reduce the severity and intensity of your migraine attacks. 

🤸‍♀️ Vigorous exercise could be a trigger for a small percentage of people with migraine, but overall benefits far outweigh the risk. If this might be true for you go slow. Consider stretching, yoga or gentle core strengthening exercises indoors to avoid overheating.

🏊‍♀️ Movement can also boost your mood by releasing endorphins (your bodies natural painkillers), boost your self-confidence, reduce stress, increase oxygen to your brain (creating happy mitochondria), helping you sleep at night and make you feel stronger. 

🏋️‍♀️ For those with Vestibular Migraine, movement is vital in helping your brain learn to compensate and recover.

👂 Like we discussed yesterday, those with migraine (especially chronic migraine) can feel this way even between attacks although it becomes worse with attacks for many of us.

👂 A range of sounds and noises become unbearable and painfully loud. In addition, sounds with movement like bass coming from the car next to you in traffic are particularly tough for those with vestibular migraine. 

👂 Although different from hyperacusis, repetitive noises like chewing, water dripping or a ceiling fan pull tinking against a light shade over and over can be unbearable for those with migraine as well.

💡 We often hear about migraine sensitivity to l 💡 We often hear about migraine sensitivity to light, fragrance, noise and even movement for some with vestibular migraine. Do those with migraine have a faulty integration system? This is a valid topic beginning to gain traction.

💡 Dr. Teixido, board certified neurologist at ENT & allergy of Delaware says (the brains of) people without migraine disease can adapt to sensory stimulus such as barometric pressure changes, light, noise, strong smells, movement etc. within minutes. Effectively turning stimulus off and moving it into the background. But for those of us with migraine our brains don’t easily adapt.

💡 And outside sensory input isn’t the only troublemaker here. Physiological stimulus like hormones, illness, sleep, stress, hunger, dehydration, exercise and pain are difficult for us to regulate as well. The irritation of the stimulus continues to build until it ends in a migraine attack.

💡 This is the reason many of us retreat to a quiet, dark room during our worst attacks. We need to turn off sensory input and soothe our sensory integration systems to effectively abort attacks.

💡 Our faulty sensory integration system & hyper responsive migraine brain causes multi sensory symptoms, not just a headache.
